Preferred Provider Organization: This is the most costly but offers the most flexibility in choosing your doctor and receiving treatment. PPO physicians agree to charge the insurance company a reduced amount in exchange for being part of the network. Because they are paid based upon services rendered, they have incentive to provide treatment.

Health Maintenance Organization: These are the most affordable plans in which a doctor gets paid a flat fee every month to provide care to all of the patients for which he/she is the primary care physician. For those HMOs that employ its own doctors (eg: Kaiser), the doctor is salaried and only treat members of the HMO. Because they do not get more money for providing more care, the incentive is to reduce the costs of care by keeping to a minimum the amount of care given.

Major Medical: Unlike HMOs and PPOs which cover doctor visits and routine medical costs, major medical policies only cover for major medical expenses such as hospitalization or surgery. This option is usually less expensive than PPOs or HMOs and is designed for those people who are willing to pay for the day-to-day type medical expenses and are looking simply for protection from the financial catastrophe that a major illness could cause.
